Effective June 1, 2015, Portico Benefits Services is changing the administrator for ELCA disability benefits to Liberty Mutual, replacing our current administrator, Aetna. Portico is working closely with Liberty Mutual and Aetna to ensure a smooth transition and uninterrupted service for members currently receiving benefits from the ELCA Disability Benefits Plan.
Benefits provided by the ELCA Disability Benefits Plan, current monthly member disability benefit payments, and 2015 monthly employer contributions are not changing.
To help manage plan members’ claims and ensure uninterrupted delivery of monthly disability payments, Liberty Mutual will send forms to ELCA members currently receiving disability benefits. The forms are not a reapplication for disability benefits, they include authorization for release of medical records, claimant information, a W4 form, and an optional direct deposit application. Members will need to mail or fax completed forms to Liberty Mutual by June 1, 2015. If mailing, members may want to have the forms postmarked by May 26to ensure that Liberty Mutual receives them by June 1.
